This survey is intended to help determine if you should be entering a McMaster site today. Please note:


  • Please complete the self-assessment tool no more than 1 hour before you plan to come to the site.

  • This screening tool is for those entering MDCL/MUMC and/or those who have been asked to use it by their supervisor or program.

  • Your supervisor needs to have your screening results before you proceed to work. Please follow the instructions provided by your supervisor to share your results.

  • You will be able to forward your results to your supervisor by using the "send results as an email' button at the end of the screening.

  • If you cannot produce your results, you may be removed from the list of individuals who have permission to be on site.
